Applicant and Referral Forms

Use the intake application to apply to Project Me Pathways, or submit a referral for a young person who may benefit from the program.

Apply for Project Me Pathways and tell us where you need support. Let us know what you want to work on and when you can take part.

For participants and their parents or guardians.
The form asks for contact information and what the participant hopes to work on. It also asks when they are available and which programs interest them. Required signatures are included at the end.

Refer a young person to Project Me Foundation Inc. and give us the background we need to understand the situation and make a useful first contact.

For community partners and referring professionals, including people working in schools, courts, or probation offices.
The form asks why the referral is being made and what support may help. It also gathers the participant and school information needed for follow-up.
